CloudMD’s nurse navigation model connects employees to the right care with ease and simplicity and supports that patient through their healthcare trajectory. The nursing team develops a personal support system that ensures each employee gets a dedicated nurse navigator who assesses their health needs and creates a care plan tailored just for them.
Additionally, there is coordinated care where the nurse navigator links employees to the right doctors, therapists, or other health specialists, depending on what’s needed. To ensure continuous follow-up, the dynamic nursing team stays in touch with the patient, helping employees stay on track with their health goals and adjusting plans if needed. The platform is easy to use; employees can reach their nurse, book appointments, and access health resources all from one digital platform. We use a whole health approach that ensures the nursing team supports physical, mental, and social health so employees get comprehensive care in one place.
